Once Acrobat Reader is installed, there are two ways to complete the forms:

1. click on the appropriate link below to open desired form, print it out blank, and complete it using a standard typewriter, or
2. click on the appropriate link below to open the desired form. Next, click the appropriate space for the first item to be filled out, type your response, and hit the tab key to move to the next item on the form. Once the form is completely filled out, print the completed form for mailing.

In either case you may save a copy of the blank form on your local hard drive or diskette for later use or reference. It is not possible to save a completed or partially-filled-out form, so be sure you have time to finish and the ability to print before beginning to fill out the form online.
