Course Requirements

Course: Dental Assisting II Preparatory Program
  1. Completion of a NC State Board-approved intraoral radiography course.
  2. You must have worked for 2 years (in the last 5 years) as a full-time chairside assistant.

All participants must adhere to the UNC Integrity Code and the Professionalism Standard.

Course: Coronal Tooth Polishing
  1. Course participants must be working as a Dental Assistant II* and be at least 18 years of age.

  2. A Request and Consent Form states that you agree to be both a patient and a participant in this continuing education course. Click Here for Form

  3. Employment and Professional Liability Insurance Coverage form that states that you are employed in a professional practice and that you have insurance coverage. This form will also need the signature of your dentist/employer.** Click Here for Form

  4. Medical History Update that will provide UNC School of Dentistry your dental healthcare. Course faculty will review your medical history prior to the clinical portion of this course. Click Here for Form
  5. The verification of Dental Assistant II status form that states that you are eligible for Dental Assistant II status according to the NC State Boards requirements and therefore can polish teeth. This form will also need the signature of your dentist/employer. Click Here for Form

Note: The above forms must be completed and submitted to the UNC CDE office prior to the start of the course.
